Experteer Overview
In this role you support Wisconsin IRIS participants by helping them navigate a self-directed long-term care program. You’ll work remotely with participants in their homes and communities to identify goals and access local resources. You build trusted relationships, educate on budgets and plans, and ensure compliance with program rules. You’ll collaborate with a cross-functional team to implement personalized IRIS plans and enable meaningful community participation.

Responsibilities
- Meet IRIS participants in person at least four times per year, including one annual home visit
- Provide program orientation about rights, responsibilities, and paperwork
- Explore participants’ life goals and community connections to develop outcomes within IRIS budget
- Document orientation, planning, and progress in the WISITs data system within 48 business hours
- Research and share community resources and supports aligned with participant outcomes
- Track plan modifications and budget-related paperwork (Budget Amendment/One-Time Expense) as needed
- Educate participants on interpreting monthly budget reports and act as liaison between Fiscal Employer Agency and IRIS Consultant Agency
- Develop engaged relationships and communicate program changes and compliance effectively
- Maintain confidentiality and HIPAA compliance
- Collaborate with internal staff to ensure successful plan implementation and attend meetings
Key requirements
- At least 2 years in health care (preferably care coordination) and 1 year serving IRIS target groups
- Bachelor’s degree in a related field (or four years of commensurate experience)
- Valid driver’s license, reliable transportation, and adequate auto insurance for job-related travel
- Ability to work independently and develop positive relationships with diverse stakeholders
- Knowledge of long-term care programs and self-determination principles
- Strong problem-solving, time management, and prioritization skills
- Excellent verbal and written communication; proficient in Microsoft Office
