In this role you support the Patient-centered Medical Home (PCMH) model by coordinating care, planning and monitoring health services for defined populations. You will work on on-site, team-based care with physicians and other providers to improve health outcomes through self-management and care planning. Responsibilities

Support PCMH care coordination and team-based care
Assess, plan, implement, coordinate, monitor and evaluate healthcare options to improve health status
Follow PCC workflow for utilization and self-management support
Participate in PCMH activities including discharge follow-up, care planning, and communication between care providers
Collaborate with patient and family/caregivers to establish mutual goals and self-management tools
Provide outreach, disease management, and education for patient-centered goals
Identify and mitigate barriers to improved outcomes
Monitor and evaluate community resources to meet health needs
Conduct telephonic or in-practice outreach, disease and case management, and related activities within licensure scope
Develop nursing treatment plans and interventions in collaboration with other providers
