In this role you support case management by screening elective inpatient admissions, coordinating discharge planning, and guiding transfers and resource use. You will ensure regulatory compliance and collaborate with nurses, physicians and external facilities to optimize patient flow. You help educate patients and families about discharge follow-up and support transfer and discharge processes. This position emphasizes professional communication, accuracy, and timely problem-solving within a patient-centered care setting.

Responsibilities

Perform initial screening of elective inpatient admissions for appropriateness and level of care
Coordinate and supervise discharge planning and transfers, including third-party contracts and ground transportation
Educate patients, families, and staff on discharge follow-up and education
Document admissions and forward information to Case Managers as needed
Utilize InterQual screening criteria for admissions and observation status
Collaborate with departments to maintain patient flow during admission and transfer
Support case managers with patient transfers and discharges
Prioritize work activities and adhere to hospital policies and procedures
Ensure compliance with federal and state Case Management regulations
Communicate professionally with patients, families, and healthcare providers
Maintain confidentiality of sensitive information
Monitor and report safety issues and equipment failures
