Senior Specialist / Project Manager

In this role you will serve as a local member advocate, helping vulnerable individuals navigate health care needs and access community resources. You will collaborate with the health care services team to provide non-clinical support in the field, including home visits and outreach at community sites. Your efforts will focus on maximizing member benefits, maintaining Medicaid eligibility, and connecting members with social services to support their overall well-being. This position offers meaningful frontline impact as you help improve access, quality, and cost-effectiveness of care.

Responsibilities

Advocate for and assist members in managing health care needs
Support the health care services team with paraprofessional duties in the field (home visits, shelters, provider offices)
Help members maximize health plan benefits, including scheduling appointments and arranging transportation
Assist members in accessing social services and community resources
Reach out to locate and support disconnected members with special needs
Conduct outreach and research to contact hard-to-reach members using internal data and community locations
Participate in ongoing or project-based outreach activities (telephonic and/or face-to-face)
Guide members to maintain Medicaid eligibility and identify financial resources

Key requirements

At least 1 year of health care experience, preferably with underserved or special needs populations
Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job-related travel
Ability to multitask while communicating with members
Excellent customer service, organizational and time-management skills
Willingness to learn additional lines of business, programs, and software systems
Excellent verbal and written communication skills
Proficiency in Microsoft Office and applicable software
