In this role you support care management and care coordination for members with high-need potential, partnering with a multidisciplinary team to deliver integrated, quality, cost-effective care. You drive implementation and monitoring of care plans, coordinate waiver services, and promote service integration across behavioral health and LTSS. You will work across settings to remove barriers and respond to changes in member needs, contributing to the broader care strategy. Responsibilities

  • Complete comprehensive member assessments within regulated timelines and conduct in-person visits as required
  • Coordinate waiver enrollment/disenrollment processes
  • Develop and implement care plans with members, caregivers, and health professionals
  • Monitor care plan effectiveness, document interventions, and adjust goals accordingly
  • Promote service integration across behavioral health, LTSS, and community resources
  • Assess medical necessity and authorize waiver services
  • Evaluate benefits and advise on funding sources
  • Facilitate interdisciplinary care team meetings for service decisions
  • Apply motivational interviewing and Molina clinical guideposts in member interactions
  • Identify barriers to care and provide coordination to address psycho/social, financial, and medical obstacles
  • Identify critical incidents and create prevention plans
  • Collaborate with licensed care managers/leadership as needed
  • Travel 25-40% as required by state/contractual requirements

Key requirements

  • At least 2 years of health care experience including LTSS or equivalent
  • LPN or LVN license (active/unrestricted if required by state)
  • Bachelor’s degree in a health care field may be required in some states (unknown if required)
  • Valid driver’s license and reliable transportation
  • Knowledge of community resources
  • Ability to work independently and in diverse settings
  • Strong communication, time-management, and problem-solving skills
  • Proficiency with Microsoft Office and navigating online portals/databases
  • Experience in care management or in medical/behavioral health settings
  • Preferred: CCM, LVN/LPN license active
  • Preferred: experience with waiver populations
