In this role, you implement and sustain a structured bereavement program within hospice care, supporting patients and families through grief for at least a year after loss. You collaborate with the Interdisciplinary Team to assess risk, tailor care plans, and coordinate community resources. Responsibilities

  • Participate in the IDT to develop and implement the plan of care
  • Collaborate with the hospice social worker on risk assessment and care planning
  • Promote bereavement risk assessment throughout care
  • Develop and execute the bereavement plan of care after patient death
  • Identify needs of non-family caregivers and ensure their bereavement support
  • Advise on intervention needs for other IDT members
  • Communicate patient and family needs to IDT
  • Educate IDT members about bereavement services
  • Prepare reports and materials to demonstrate program value
  • Deliver bereavement support using internal resources or community partners
  • Organize and run grief support groups
  • Maintain a bereavement resource library
  • Coordinate with Volunteer Coordinator on volunteer involvement
  • Attend community events to promote Compassus and bereavement services
  • Develop and deliver memorial services for program patients and nursing facilities
  • Participate in funeral or memorial services if requested
  • Perform additional duties as assigned

Key requirements

  • Bachelor's or higher degree in Counseling, Social Work, Psychology, Theology, Divinity, or equivalent strongly preferred
  • Minimum of two years of experience in grief, loss, or bereavement support strongly preferred
  • Clinical background in death, dying, and bereavement is advantageous
  • Equivalent combination of training, education, and experience considered
  • Proficiency in English; strong written and verbal communication skills
  • Flexible schedule including nights and weekends
